Doctor’s Note: Your Essential Guide to Microneedling

When deciding if microneedling is right for your particular skin, there's many options and a confusing world of pens, rollers, topicals, PRP, vampire facials, it's too much! We've tried to break down the most popular microneedling treatments available to help you sift through this information.

What is Microneedling?

Microneedling is a skin treatment that uses tiny, fine needles to create microscopic punctures in the skin. These tiny “wounds” stimulate the skin’s natural healing process, encouraging the production of collagen and elastin. Collagen is a protein that helps keep the skin firm and smooth, while elastin allows the skin to bounce back after being stretched.

By boosting the production of collagen and elastin, microneedling can help improve the skin’s texture, reduce the appearance of scars, and smooth out fine lines and wrinkles. The procedure is typically done using a small device with multiple tiny needles that are rolled or stamped across the skin.

Microneedling can be done on various parts of the body, but it’s most commonly used on the face, especially in areas with acne scars, fine lines, or uneven texture. The procedure is safe for most skin types and can be customized to treat specific concerns.

How Does Microneedling Work?

Microneedling works by creating controlled, tiny injuries in the skin’s surface. When the skin heals from these micro-injuries, it produces new collagen and elastin, which improves the skin’s structure and appearance. This process is often called "collagen induction therapy."

How deep do we go? It depends on your skin and the condition we’re treating! Generally, microneedling pens can penetrate the skin from 0.025mm to 2.5mm. We usually use up to 1.5mm on the face, as this is deep enough to cover the entire dermis, even deep wrinkles!

Benefits of Microneedling

Microneedling has many benefits for the skin. It can help treat a variety of skin concerns, including:

  • Improved Skin Texture: Microneedling helps smooth rough, uneven skin by promoting collagen production. This makes the skin feel softer and look more even.

  • Reduced Acne Scars: The treatment can break up the scar tissue and promote healing, making acne scars less noticeable over time.

  • Fading Fine Lines and Wrinkles: By stimulating collagen production, microneedling can help reduce the appearance of fine lines and wrinkles, making the skin look smoother and more youthful.

  • Shrink Pores: Microneedling can tighten the skin and reduce the appearance of large pores by stimulating collagen production around them.

  • Improved Skin Tone: Microneedling helps even out skin tone by encouraging the production of new, healthy skin cells.

  • Treatment of Stretch Marks: Microneedling can help reduce the appearance of stretch marks by promoting the production of collagen and elastin in the affected areas.

  • Hair Growth Stimulation: In some cases, microneedling is used on the scalp to promote hair growth in people experiencing hair thinning or hair loss.

What to Expect During Microneedling

Before your microneedling treatment, your dermatologist will discuss your skin concerns and determine if the procedure is right for you. Here's what you can expect during the procedure:

  1. Consultation: Your dermatologist will assess your skin, ask about your medical history, and discuss your treatment goals. They will also explain how microneedling works and answer any questions you may have.

  2. Numbing: A topical numbing cream will be applied to the treatment area to ensure you're comfortable during the procedure. After the cream has taken effect, the microneedling treatment will begin.

  3. The Treatment: Your dermatologist will use a microneedling device to create tiny punctures in your skin. The treatment typically lasts between 20-30 minutes, depending on the area being treated.

  4. Post-Treatment: After the procedure, your skin may appear red or slightly swollen. This usually goes away within a few hours to a day. You may be given aftercare instructions, such as moisturizing and using sunscreen.

Microneedling, But Make It *Fancy*

So far, we've covered traditional microneedling (AKA just the basics). There's much more we can do, though! When combined with other treatments, like topical products (such as hyaluronic acid and Vitamin C), PRP (platelet-rich plasma from your blood), or exosomes (which have many growth factors), the effects can be even stronger. Adding radiofrequency and heat during microneedling can boost collagen production even more. Lastly, "Glass Skin Botox" uses microneedling with Botox to relax the skin and help improve its appearance. Each of these treatments targets different skin concerns to help you get smoother, healthier skin.

Microneedling + Topicals

Microneedling creates micro-channels in the skin, making it more permeable for ~48 hours. By applying topicals during this 2-day period, active ingredients can readily penetrate into the dermis. The microneedling itself promotes collagen and adding a topical that also promotes collagen is a win-win! Some of our favorite topicals to apply after microneedling include:

  • Hyaluronic acid: Adds hydration and firmness

  • Vitamin C: Improves tone/texture

  • Tranexamic acid: Reduces dark spots

Note: Any topical you apply after microneedling must be STERILE!! We don't want to introduce an infection into your skin!

Microneedling with PRP

Have you ever heard of a “Vampire Facial”? This is it! PRP stands for platelet-rich plasma, which is a concentration of a patient's blood that’s rich in growth factors. When combined with microneedling, the skin absorbs these growth factors to boost collagen production. You can think of PRP like fertilizer for your skin, helping it grow and heal.

PRP comes from your own blood and is packed with proteins and growth factors that help the skin heal. When applied after microneedling, PRP can promote collagen production, leading to firmer, younger-looking skin. It also speeds up the skin's natural healing process, which reduces downtime and improves results. The combination of microneedling and PRP works together to enhance skin texture and tone more effectively than microneedling alone.

Microneedling with Exosomes

Exosomes are tiny particles that carry proteins and other materials to help cells communicate. When used with microneedling, exosomes travel deep into the skin, acting like an envelope with instructions. The cells "read" the message and start making more collagen. This process boosts collagen production, making the skin firmer, speeds up healing to reduce downtime, and improves skin texture and tone. However, the downside is that exosomes are still new, not yet FDA-approved, and more research is needed.

Microneedling with Radiofrequency

Have you ever heard of Morpheus8, Profound, Secret, Vivace, or Genius? These treatments combine radiofrequency (which uses heat 🔥) with microneedling. The heat helps the microneedles reach deeper layers of the skin for more dramatic results. This combination not only boosts collagen production but also tightens the skin. The downside? Sometimes, the heat can go too deep and melt fat, which can cause a sunken look. 😱 That’s why it’s important to choose an experienced provider to avoid this!

Microneedling + Glass Skin

This isn't exactly microneedling, but it’s similar. There are devices with many small, hollow needles that push active ingredients into the skin, a process called "mesotherapy." One popular device used in the office is called "AquaGold." It’s a microneedling "stamp" with tiny gold-plated needles (0.6mm) that deliver treatments directly into the skin. What kind of treatments? We customize it to meet your skin's needs! Since it’s done in the office, we can use ingredients like Botox, hyaluronic acid, and PRP.

Microneedling Cost

The cost of microneedling typically ranges from $200 to $700 per session, depending on factors like the area being treated, the experience of the provider, and the location of the clinic. For the best results, multiple sessions are often recommended, so the total cost can add up. Some providers may offer package deals for multiple treatments, which can lower the overall cost per session.

How Long Does Microneedling Take to Work?

Microneedling typically requires multiple sessions to see noticeable results. Most people begin to see improvements after 2-3 treatments, with optimal results often appearing after 4-6 sessions. The exact number of treatments depends on the skin concerns being addressed and the area being treated.

After your initial treatments, maintenance sessions are usually recommended every 3-6 months to keep your skin looking its best.

Risks and Side Effects of Microneedling

While microneedling is generally safe, there are some potential risks and side effects. These may include:

  • Redness and Swelling: It’s normal to experience redness or mild swelling after the procedure, similar to a sunburn. This typically fades within a few hours to a day.

  • Bruising: Some patients may experience bruising, particularly in sensitive areas.

  • Dryness or Peeling: After treatment, the skin may peel or feel dry as it heals. This is temporary and should resolve within a few days.

  • Infection: Although rare, there is a small risk of infection if the skin is not properly cared for after treatment.

  • Hyperpigmentation: In some cases, microneedling may cause darker spots on the skin, especially in people with darker skin tones. This risk can be minimized by following aftercare instructions and avoiding sun exposure.

  • Scarring: While microneedling is meant to improve the appearance of scars, improper technique or care can sometimes lead to new scars.

Microneedling Aftercare Tips

Proper aftercare is essential for the best results and to reduce the risk of complications. Here are some general aftercare tips:

  • Avoid Sun Exposure: Your skin will be sensitive after treatment, so avoid direct sun exposure for at least 24-48 hours. Use a broad-spectrum sunscreen if you need to be outside.

  • Moisturize: Keep your skin hydrated by using a gentle moisturizer. This will help soothe and protect the skin as it heals.

  • Don’t Pick or Scratch: Avoid touching or picking at your skin, as this can cause irritation or infection.

  • Avoid Makeup: It’s best to avoid wearing makeup for at least 24 hours after treatment to allow your skin to heal.

  • Follow Your Dermatologist’s Instructions: Your dermatologist will give you specific aftercare instructions tailored to your treatment and skin type.
